The agreements
between
you and us.

These terms apply to all treatments, consultations and services you receive at Lunaris Clinics. They have been drawn up in line with the Dutch Medical Treatment Agreement Act (WGBO) and standard practice for independent care providers. By scheduling an appointment, you agree to these terms.

1. Definitions

  • Lunaris Clinics: the care provider, registered with the Dutch Chamber of Commerce under KvK number 98162802 (AGB code 84059198), with locations as listed on the Locations page.
  • Client: the natural person receiving a treatment, consultation or service from Lunaris Clinics.
  • Treatment: any medical consultation, intake, follow-up, prescription or lifestyle support offered by Lunaris Clinics.
  • Agreement: the medical treatment agreement between Lunaris Clinics and the client, in accordance with article 7:446 of the Dutch Civil Code (WGBO).

2. Applicability

These terms apply to every agreement between Lunaris Clinics and the client. Any general terms of the client are expressly rejected. Deviations only apply when agreed in writing.

3. Sign-up and intake

A treatment journey always begins with an intake by our GP. During this intake the doctor assesses whether treatment with Tirzepatide or another service is medically appropriate and suitable for your situation. We reserve the right to refuse treatment when it is not medically responsible.

Lunaris Clinics works exclusively on a private, self-paying basis. We do not invoice health insurers; you are responsible for any reimbursement via your supplementary insurance, if applicable.

4. Pricing and payment

  • Current pricing is published on the Pricing page and on the sign-up forms.
  • Payment is due before or directly after the treatment, by invoice or card payment.
  • In case of late payment we may charge statutory interest and (extra)judicial collection costs.
  • Pricing may be adjusted; changes are always communicated in advance.

5. Rescheduling or cancellation

  • Cancelling or rescheduling is free of charge up to 24 hours before the appointment.
  • For cancellations within 24 hours or no-shows, we may charge the full consultation fee.
  • You can cancel via your confirmation email, online via SimplyBook, or by calling our reception.

6. Client obligations

  • You provide, to the best of your knowledge, all information needed for safe treatment — including medical history, current medication and allergies.
  • You follow the given instructions and treatment plan, and contact us immediately in case of severe side effects or symptoms.
  • You are responsible for the correct use and storage of prescribed medication.

7. Lunaris Clinics' obligations

  • We provide care in line with the prevailing professional standards and the WGBO.
  • We maintain a careful medical record and respect medical confidentiality.
  • We inform you clearly about the nature, purpose, risks and costs of a treatment, so that you can give informed consent.
  • We process your personal data in accordance with our privacy statement.

8. Liability

Lunaris Clinics is liable only for damage that is the direct result of an attributable shortcoming in the performance of the agreement. In all cases, our liability is limited to the amount paid out by our professional liability insurance for the relevant case.

We are not liable for damage resulting from failure to follow our instructions, withholding medical information, or independently changing the prescribed dose.

9. Record retention

The medical record is retained for at least 20 years after the last contact, in line with the WGBO retention requirement. You have the right to access, a copy and — where legally permitted — destruction of your record at any time. See our privacy statement for the procedure.

10. Complaints and disputes

Have a complaint? Please discuss it first with the care provider involved or with our practice manager — we usually find a solution together. If we can't reach a resolution, you can turn to:

  • The independent complaints officer we are affiliated with (details available on request).
  • The Dutch Healthcare Disputes Committee (Geschillencommissie Zorg) Lunaris Clinics is affiliated with — rulings are binding.
  • For privacy complaints: the Dutch Data Protection Authority.

11. Applicable law

Dutch law applies to all agreements with Lunaris Clinics. Any disputes — to the extent not handled by the Disputes Committee — are submitted to the competent court in the district where Lunaris Clinics is established.

12. Changes

Lunaris Clinics reserves the right to amend these terms. The most recent version is always on this page; the date at the top reflects the latest update.
